Prescription for physical therapy from your physician
Arrive 15 minutes early for your first appointment to complete paperwork OR click below and print to complete the paperwork prior to your first appointment.
Wear appropriate clothing to allow access to painful areas.
Handicap parking is available. We are wheelchair and ADA compliant.
Co-payments are expected prior to every treatment session. We accept personal checks and cash only.
